Arizona Recovers: Mental Health Track
Arizona Recovers, as a community partner, is committed to lessening the burden and assisting with continued care for patients who are identified moderate to high risk for suicide through BRAVE Connections-Mental Health Track.
Patients post attempt are most at risk seven days post discharge and up to 70% never attend their first outpatient appointment (Knesper, 2010). It has been found that emergency department interventions could reduce annual deaths from suicide by 20% (National Action Alliance Suicide Prevention: Research Prioritization Task Force, 2014).
Eligibility & Goals
The Goals of the program are to:
- Reduce readmission to Emergency Department
- Improve compliance with follow up care
- Increase in personal coping and resiliency skills
Peer Support Program
How It Works:
- “Caring Contacts” post discharge for a year including phone calls and postcards.
- Peer support program post discharge for a year utilizing motivational interviewing and cognitive behavioral techniques and focusing on developing resiliency skills.
- Connection with patient and/or family virtually within Emergency Department or request services here.
